Before meeting with your funeral director to make arrangements, please review the following list of information that will be needed:

  • Obituary information
  • Clothing (to include undergarments)
  • Photograph
  • Social Security number
  • Deceased's parent's names (to include mother's maiden name)
  • Date of birth, City, and State
  • Military discharge paperwork (DD-214 Form)
